U.S. EMPLOYER:

1. Proposed job title to be held in the U.S.

2. Proposed work address in the U.S.

3. Proposed annual salary and any benefits. Please also indicate if the salary will be paid by the U.S. or the foreign employer;

4. Estimated start date

5. Detailed job duties to be held in the U.S.: Please provide as much specifics as possible including the title(s) of persons who will directly report to the individual as well as the title(s) of persons who the individual will report to. Please also include any budget figures the individual will oversee, and whether the individual will have the authority to hire or file subordinate personnel

6. Organizational Chart identifying the proposed position with name(s) and title(s) of supervisors and direct reports, if any.

FOREIGN EMPLOYER:

12. Completed Immigration Questionnaire (KIP form attached). Please be sure to provide as much detail regarding your current responsibilities and the title(s) of anyone you directly supervise

13. Photocopies of individual's passport pages and any prior U.S. visa(s)

14. Photocopies of individual's university or trade school diplomas and certificates, with course transcripts

15. Photocopy of most recently available governmental income tax withholding form (U.S. "W 2" Wage and Tax Withholding Statement, Canadian "T 4," British "P60" or Australian "Group Certificate", or similar foreign tax authority document) showing year, name of foreign employer, name of employee, salary paid, tax withheld, etc.

16. Individual's resume, curriculum vitae, etc. – Please make sure you provide details regarding your current and previous experiences
