Please also include any budget figures the individual will oversee, and whether the individual will have the authority to hire or file subordinate personnel
  1. Organizational Chart identifying the proposed position with name(s) and title(s) of supervisors and direct reports, if any.

  1. Completed Immigration Questionnaire (KIP form attached). Please be sure to provide as much detail regarding your current responsibilities and the title(s) of anyone you directly supervise
  1. Photocopies of individual's passport pages and any prior U.S. visa(s)
  1. Photocopies of individual's university or trade school diplomas and certificates, with course transcripts
  1. Photocopy of most recently available governmental income tax withholding form (U.S. "W 2" Wage and Tax Withholding Statement, Canadian "T 4," British "P60" or Australian "Group Certificate", or similar foreign tax authority document) showing year, name of foreign employer, name of employee, salary paid, tax withheld, etc.
  1. Individual's resume, curriculum vitae, etc. – Please make sure you provide details regarding your current and previous experiences
